Gut Hub Self-Registration
To start the registration process, first, complete the assessment questionnaire. Your answers will be read by the clinical team to make sure that the Gut Clinic meets your needs. Once the clinical team has completed triage (please allow 3-5 working days), our lovely admin team will be in touch to book you in. As part of the booking process, we ask that you complete a 3 day food diary, an initial score questionnaire and the relevant consent forms. Please also let us have a copy of relevant past test results and hospital correspondence (e.g. endoscopy report, gastroenterology letters)Voila’ …it’s done!
